NDIS Community Nursing Care
NDIS Community Nursing Care is available to eligible NDIS participants whose NDIS Plan includes community nursing capacity building.
This service provides assistance with general nursing care to maintain your health, wound management, chronic condition management, and hospital-type care outside of the hospital.
Our nurses can also assist you in treating any existing health issues. You can get in-home nursing care from one of our registered nurses.
Our Support Services
Our goal is for our nurses to help you live independently in your own home while receiving excellent clinical care. Nursing tasks available include the following:
- Medication administration & supervision
- Assessment of medications and supplement usage
- Tube feeding assistance
- Catheter care and management
- Wound care
- After hospital stay follow ups
- Diabetes management
- Palliative care support
